SUPPORT ELIGIBILITY ASSESSMENT FOR TRANSITIONAL PLANNING
Transition planning is essential for individuals diagnosed with autism spectrum disorder and/or intellectual disability. Commencing transition planning as early as age 16 is recommended, as it allows for adequate preparation for adulthood before reaching 19. This proactive approach can assist in determining eligibility for important support services such as Community Living BC (CLBC), Persons with Disabilities (PWD), or vocational training programs.
The assessment process varies depending on the time elapsed since the previous assessment identifying neurodevelopmental disorder(s).
ASSESSMENT PROCESS MAY INVOLVE:

Adaptive functioning interview with parents or caregivers

Interview with the client

Intellectual functioning evaluation

Academic functioning evaluation (when requested and if appropriate)

File review, scoring, and preparation of assessment report

Completion of CLBC support or PWD forms

Feedback session
